Integrated Care for the Geriatric Patient aims at the coordinated management of chronic illness, allowing better monitoring and preventing visits to the Emergency Department.

Includes:

• Identification and clinical evaluation of the patient;
• Structuring an individual care plan favoring the provision of care at home;
• Provision of health care integrated in a multidisciplinary team, whose main function is to carry out home visits and promote the connection between the various care providers.
